Stretch marks (striae) result from rapid skin stretching during pregnancy, growth spurts, weight changes, or anabolic steroid use, causing the dermis to tear at a microscopic level. They progress through phases: red/purple (striae rubrae) when fresh and active, then mature to silvery-white (striae albae). Treatment is significantly more effective during the red phase — older white striae are much harder to improve.

Multiple modalities exist: fractional CO2 laser, RF microneedling (Morpheus8 is popular for this), pulsed dye laser (effective for red striae specifically), microneedling with PRP, and topical retinoids. Best outcomes typically combine modalities — for example, fractional laser + topical retinoid + occasional PRP. Patient education about expectations is critical: complete erasure of stretch marks is rare; substantial improvement is the realistic goal. Treatment courses typically run 4-8 sessions spaced 4-6 weeks apart over 4-8 months.
