Cheek implants restore or enhance midface volume by placing a solid implant over the malar (cheek) bone, accessed through a small incision inside the upper mouth. Common materials include silicone, porous polyethylene (Medpor), and ePTFE — each with different tissue integration profiles. The procedure is appropriate for patients with naturally flat cheekbones, age-related midface volume loss, or those who've found temporary fillers don't deliver the projection they want.
Modern surgical planning uses 3D imaging to size and position implants symmetrically, and revision rates have dropped significantly with improved technique. Recovery involves 1-2 weeks of swelling and a soft-food diet during the first week. Once integrated, results are essentially permanent — though the surrounding face continues to age normally.
