HIFU (High-Intensity Focused Ultrasound), branded by various devices including Ultherapy, delivers focused ultrasound energy to precise depths below the skin to trigger collagen remodeling. Unlike radiofrequency or laser, HIFU can reach the SMAS (Superficial Musculoaponeurotic System) — the same fibrous layer surgeons tighten in a traditional facelift — without disturbing the surface of the skin.
Energy is delivered through a transducer placed on the skin, creating thermal coagulation points at three depths (1.5mm, 3.0mm, and 4.5mm) corresponding to dermal, subdermal, and SMAS-level tissue. The body responds with a months-long collagen-and-elastin remodeling cascade. Most patients see visible lift improvement at 2–3 months, peaking at 6 months, with results lasting 12–18 months.
Common treatment zones include the brow, mid-face, jawline, jowls, and submental area. HIFU is most effective on patients with mild-to-moderate skin laxity (typically 35–65 years old). It does not replace a surgical facelift for severe laxity but offers meaningful tightening for the majority of patients who aren't ready for surgery.
